Not so Typical NAION

Case of the week:

A healthy female in her 50s presents with painless vision loss in the left eye. She had an altitudinal visual field defect in the left eye and optic nerve swelling with a peripapillary hemorrhage.

Initial Diagnosis prior to referral to Neuro-Ophthalmology: NAION


Actual Dx: 2 cm massive ICA supraclinoid aneurysm compressing the left optic nerve.

The patient was sent to the hospital and had an emergent aneurysm coiling avoiding a catastrophic outcome.
